Our simple process

1.

See if you’re eligible

Call us to schedule a nurse to visit you at home to determine if you’re eligible for the program. You’ll find out how many weekly hours of care you can get. Your Medicaid plan completely covers the cost!

2.

Choose a caregiver

You choose a caregiver: your daughter, son, relative, or friend. If a friend or family member is not available to be your caregiver, we will match you with a certified, reliable, compassionate Home Health Aide or Personal Care Assistant.

3.

Get loving care

Choose a home care schedule that is convenient for you. Receive ongoing care from a person who truly cares for you. Live happily and feel comfortable knowing that your health is in good hands.

CDPAP vs HHA/PCA

What’s the difference between CDPAP services and Home Health Aide / Personal Care Aide services?

CDPAP

CDPAP (Consumer Directed Personal Assistance Program) is a New York State Medicaid program that allows you to choose your caregivers. If you’re eligible, you can choose a daughter, son, relative, or friend to care for you – and they’ll get paid up to $21.09 per hour.

Friends & Family offers two kinds of caregivers to help you live easier – and happier. These are trained and certified professionals hired and placed by us. Your level of need will determine if you are eligible for a home health aide or personal care aide.
